Arriving prepared helps us provide the best possible care while making your appointment as efficient and comfortable as possible. We recommend bringing the following items with you:

  • Your vision and medical insurance cards
  • A valid photo ID
  • Your current glasses and/or contact lenses
  • A list of current medications, if applicable
  • Any questions or concerns you'd like to discuss with your doctor
